NE 67 application

I'm about to submit my application to the NE 4000 footer club. I'm already a member of the NH 4000 footer club. Do I have to re-submit the list of dates and comments of them again or just the application with the remaining peaks in Me. and VT.? FYI, finished on No. Brother in Aug. Also B/W over to Fort and the crash site. Now nursing a torn meniscus suffered in Nov.

if you are on file for the nh 4000's just send the application with the new england peaks outside of new hampshire. i finished this new england list this fall and that was all i needed to send in for recognition.
