Forwarding lists to AMC?

I have been logging the dates I climbed winter peaks in the space provided in the back of the AMC guidebook. Can I photocopy that and mail it to Eric in Exeter. Or to I need to request a form from the AMC first, and if so who do I request the form from? I have a form, I think, I received from the AMC Boston office a few years ago on which I have recorded my summer 48, but I never sent it in. Does it matter how I record the peaks as long as it's legible? Thank you.

When you are typing your response, just click on the little icon that looks like a globe with a small chain (or figure eight) below it. It's to the right of the little icon that looks like a mountain scene. It'll pop up a dialog box. Type in the text, then type in the URL, and there you are.
